Transcript Requests

How can I obtain a copy of my transcript? Your transcript is an official document that is typically sent directly from the Counseling Office to employers and colleges.

• If you are a current student: complete the CURRENT STUDENT - Transcript Request Form and submit it to Ms. Oldfield in the Counseling Office located in the Main Office of the High School.

• If you are a former student/graduate of Dansville High School: please print out the FORMER STUDENT TRANSCRIPT REQUEST FORM and send the completed form to Ms. Amy Oldfield, Dansville HS Main Office, 282 Main St, Dansville, NY 14437.
